Migrant Outreach Worker, Cabarrus Rowan Community Health Centers

Summary of Position:

The Migrant Outreach Worker is responsible for completing basic health assessments, outreach, medical case management, referrals, and follow-up to migrant and seasonal farm-workers and their families. She/he must be able to read, write and speak English and Spanish fluently. This position requires flexible hours (including evenings), a valid driver's license, reliable transportation, and sufficient vehicle insurance to cover driver and passengers to
provide support and outreach to our Migrant and Farmworker communities.

Minimum Qualifications:

Excellent oral and written communication skills. Ability to effectively develop and nurture relationships with a diverse group of stakeholders. Must be able to travel with personal car.

  • Experience: Training/experience in health field and with minority community Preferred and One(1) year experience with farm-worker related issues Preferred

  • Additional skills required: MUST BE BILINGUAL AND PROFICIENT IN SPANISH TO ENGLISH AND ENGLISH TO SPANISH TRANSLATION - THIS IS A REQUIREMENT FOR THIS POSITION. Must have excellent verbal and written communication skills (in English and Spanish).

  • Education: High school diploma or GED

  • Ability to establish and maintain positive, effective, professional relationships with patients, providers,

    coworkers, and superiors, state and local agencies, growers, and clients

  • Possess knowledge of modern office equipment and Microsoft Office software programs

  • Basic computer skills; must be able to quickly learn to use Athena electronic health record system to

    accurately complete the required documentation in patients' records
     

Certification(s)/Licensure: Valid driver's license, car insurance and reliable transportation

The physical demands described here represent those that must be met by an employee to successfully perform the essential functions of this job.

· Current driver’s license and access to reliable transportation and willingness to travel when necessary
 

Key Responsibilities:

1. Completes health assessments with farmworkers at farmworker locations to ascertain any service needs.

2. Setting up and taking down mobile medical clinics at outreach sites –at migrant camps, camps and other gathering sites

3. Assists with coordination of outreach activities including affiliated projects

4. Disseminates information to farmworkers regarding clinic services and state and local public services.

5. Maintains outreach record-keeping system and assures accurate documentation on all forms.

6. Provides case management services (e.g., assessment, referral, follow-up, and re -assessment). May provide direct services (e.g., food, clothing, and over-the-counter medications).

7. Participates in health center/clinic staff meetings to provide input.

8. Works in accordance with NC Farmworker Health Program grant requirements.

9. Maintains computer management data system for purposes of program tracking and data reporting

10. Ensure culturally and linguistically appropriate services.

11. Provide structured patient education on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals.

12. Develop, discover and attend community events in order to promote coverage options and the mission and services of the organization.

13. Collaborate with various local organizations to build awareness of coverage options, spur enrollment, and build referral linkages.

14. Represent the company professionally and positively to enhance and promote the core values and mission of the organization.

15. Accurately provide required reporting to track goal achievement and client satisfaction.

16. Assist in the development and implementation of organizational outreach and enrollment initiatives.

17. Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.

18. Perform other duties as assigned.
