|These temporary positions will be assigned to the Department of Health & Human Services - Office of Rural Health. The Community Health Worker (CHW) Coordinator will have primary responsibility for monitoring “Targeted Support for Rural and Underserved Communities Support for Patients with COVID Related Needs through Community Health Workers” contracts awarded through the Office of Rural Health. The position will manage activities in support of Community Health Workers in NC for COVID-19 related services. Specifically, the successful candidate will ensure that CHWs – hired through selected “Targeted Support for Rural and Underserved Communities Support for Patients with COVID Related Needs through Community Health Workers” Contractors - provide linkages to NCCARE360, align contact tracing activities with Local Health Departments through the COVID Community Team Outreach (CCTO) tool, and provide telehealth services for residents when applicable. |
The CHW Coordinator will serve as a liaison between contracted entities and the Department to ensure that evaluation criteria and fiscal compliance are met. The Coordinator will monitor contractor activities to ensure they are focused on a primary care patient population as well as service industry employees, Skilled Nursing Facility/Long-Term Care employees, and historically marginalized populations. The coordinator will also interface with other key stakeholders such as state agencies, professional medical and health care societies, and selected community groups that provide social services support.
The CHW Coordinator will work with the contractors to ensure CHWs who are hired will use evidence-based models of care in community-based, community-informed and culturally appropriate ways to support marginalized and vulnerable communities.
