Farmworker Outreach Specialist - Surry Medical Ministries

Bilingual (English/Spanish) Speaking REQUIRED

JOB SUMMARY

The Lead Farmworker Health Outreach Specialist acts under the direction, guidance, and supervision of the Farmworker Health Outreach Coordinator. This position is responsible for completing and documenting basic health assessments, referrals, and follow-ups to migrant, H-2A, seasonal, and retired farmworkers. Additionally, outreach activities, case management, and health education are core expectations of this position. A teamwork approach is critical and assisting interns/students is expected. The position requires flexible hours (evenings and weekends). A valid NC driver's license, car insurance, an acceptable driving record, and reliable transportation are necessary.

PHYSICAL DEMANDS/ WORKING CONDITIONS 

This position requires frequent sitting for prolonged periods and operating standard office equipment such as computers, telephones, fax machines, and copiers. It may require lifting 20-30 pounds. It requires normal vision range, hand-eye coordination, and manual dexterity. Work is performed in an office environment, medical and community settings. Community outreach is typically done outside and must withstand all climate variables (e.g., heat, cold, rain, farmland, etc.). Must be able to work flexible hours, as evening and weekend work are necessary. This position requires contact with staff, external clients, and vendors and considerable driving to remote locations. Must be able to work in a team environment or without supervision. A valid driver's license and proof of insurance are required for this position. It is an expectation for this position to drive a company vehicle to transport patients and perform outreach duties, so maintaining a driving record in compliance with Surry Medical Ministries insurance policy is a requirement.  

MINIMUM QUALIFICATIONS

Education: Minimum requirement is a high school diploma 

Experience: Two years of experience in community service. Public health outreach is preferred.

Current/valid License: NC Driver's License Required
